From a game? Kefka (FFVI) wins hands down. As for anime, Jinnai (might have spelled that name wrong) from any incarnation of El-Hazard is excellent. Kind of similar laughs, actually....
Evil is hard, but the laugh that scared me the most was Quatre in the dubbed version of Gundam Wing - and it's not much easier to listen to in the Japanese version, either. That just creeps me out. I've boycotted the end of that episode. o.O
When I think of evil laughter, I picture big-breasted women, like Emerald in Sailormoon. >.> The Mwa-ha-ha-HA~! laughers. But they're kinda funny after a while...

Not sure if they are so much evil as completely rediculous, but Jinnai from El Hazard the Wanderers, and Naga from the Slayers movies...oi..they will drive you nuts!
