Em, just go to a video that has ops on it - most creators make one or two of them public. There's a button for 'more opinion info' on there - for a reason.
As far as how to op - everyone has their own standards, really.

If you'd like an example of what I'd consider two truly exemplary ops, here:

Here are two of my favorite reviews (though I could link more if you want other examples):

That's the only review I've received from Wonka, but he's very detailed, pointing out all sorts of things I didn't notice. And SSJVegita is the person I ended up modling my own reviews after (his breakdown).
Usually I give my first impression - things that I liked the first time through for the 'good' box. Then I note anything that stood out (not specific scenes) as being a problem in the 'bad' box. Then, for the 'general' I do a breakdown. I explain each score and how I decided on that score. If I gave a low score for action sync, I'd point out the reasons under that 'title' in the breakdown, whether it's specific scenes or general likes/dislikes. I do that for all of the different scores.
For Overall, I don't go by the opinion-guide. I go by the little bit of text phade had on the actual 'submit opinion' sheet - the ones that said 10 = can't get any better, and 1= should never have been made. To me, that means the 'overall' is a score of how much improvement could be done to the video (how far it is from the perfect 10 where it can't get better). I don't consider it just an average of the other scores. After all, if only one thing is wrong with the video (a 5 for capture quality), that's not hard to improve, so it wouldn't take much for the video's overall to hit a 10. If I went by the average, I'd be counting off a lot for a very easily fixed problem (like running deinterlace).
Note, when I point out things I didn't like, especially technical things, I back it up by telling how to fix those problems. Some see reviews as a way to make creators better. Me, I see them as feedback - and help, if the person wants/needs it. So it's fitting to explain the scores because numbers don't really tell you that much. If they did, we'd just have the star scale.

Here are a couple ops I thought were pretty good. They both broke down the op and covered why they scored each catagory the way they did which is so much more helpful than 'the video sucked' or some other vauge phrase like that. I especially like that in one of the ops they gave specific time stamps for what they were talking about so I can go back and be positive I know what he's refering to. To some degree I've tried to style my own ops on these.
