Forgive me for missing this if it's clearly listed somewhere, but I can't seem to find the appropriate information...
The entry form is necessary to enter the Pro contest. Fine and dandy here; no problem. However, the only mention I see of it with regards to sending it along is if you're sending in DVDs to have burned and sent back to you:

PDF, scan, sign with a tablet, or chickenscratch it with MSPaint, same diff as long as it's signed. It's a liability thing, courtesy of the (not so theoretical) possibility of mature content on the discs.
