If this is a reference to working with VOBs directly via AVISynth, then EADFAG does cover making lossless AVIs or using the swap method instead, both of which avoid these.

I think the question was more along the lines of "why did the Org staff approve the submission of a guide that contradicted one they already had endorsed?", not "why did <i>you make</i> a guide that contradicted one the Org had already endorsed?".
