Hm, one of the most disturbing scenes I've seen in an anime was in Ghost in the Shell, the movie, where the main character jumps on top of the tank-like weapon and tries to pull the hatch off. You can see every muscle in her back straining and eventually tearing apart under the stress she puts on her body. Kind of disgusting, but...strangely fascinating.

I really, really would like to see Chrono Crusade.

And just off the top of my head, the scene in the Escaflowne movie where Folken breaks each of Dilandau's fingers made me cringe.
There are a few scenes from Princess Mononoke, such as when Moro's head kind of "shuffles" and then bites off Lady Eboshi's arm, and the whole scene with the mutalation of the forest spirit.
The end of the Rurouni Kenshin OVA - Seisouhen was very upsetting as well as Nuriko's death in Fushigi Yuugi - very dramatic
Full Moon wo Sagashite got quite emotional during that last bit of episode 51 and all of episode 52. Even though the ending was fairly happy, I could almost get teary eyed just listing to the song "New Future".
I havn't seen Grave of the Fireflies, but I have seen a few music videos that made me cry ^^;

Let's see . . . how about in Perfect Blue when that guy gets stabbed in the crotch with the ICE PICK and is then stabbed to death?
Also, in Akira. When Tetsuo begins hallucinating and his stomach explodes open dumping his intestines onto the street? Or when Tetsuo's girlfriend gets crushed in his own disgusting mutated body?

Another really freaky scene in Chrno Crusade is when Aion puts Chrno's horns in Joshua and it makes him go all crazy insane and he goes around blowing shit up and freezing moments in time so he can stop the "noise".

Something that i still cant get over is in Strawberry Eggs. in the last episode when the Principal tells Ba chan that they once had a teacher like that (reffering to the crossdressing guy who's name escapes me right now), and Ba chan replies something along the lines of "I know, and he's been watching all the time" or something like that.
In other words Ba chan is a man
