This isn't true if you use avisynth as it generates all the I-frames on the fly. Even without avisynth this isn't true. Ask all of the people who use crappy DIVX fansubs as a source.
No, without AVISynth, it is correct. Unless you cut between the last frame in a GOP and an I-frame, you are not guaranteed that the cut will be precise.

What you can do to get around this is index the MPEG file. DVD2AVI does this when it generates project files. mpeg3toc is a GNU/Linux (doesn't run on other Unices all too well afaik) tool which does the same.

There was a 4gb size limit on fat32 hard drives. NTFS formatted drives don't suffer from this.

The limitations are based on the filesystem you use.

NTFS:
Maximum file size

Theory: 16 exabytes minus 1 KB (264 bytes minus 1 KB)
Implementation: 16 terabytes minus 64 KB (244 bytes minus 64 KB)

Maximum volume size

Theory: 264 clusters minus 1 cluster
Implementation: 256 terabytes minus 64 KB ( 232 clusters minus 1 cluster)

BTW the numbers in the parenthesis should be as follows:

2^64 bytes minus 1 KB
2^44 bytes minus 64 KB
2^64 clusters minus 1 cluster
2^32 clusters minus 1 cluster

They were subscript on the site I copied them from.

The maximum filesize on an NTFS filesystem is roughly 16 terabytes.
