Watch a little more and you'll realize that the transformation sequences are not in real time. A good example is Chibi-moon's transformation while in the black dream hole in the SuperS movie. It takes a grand total of about 2 seconds for her to transform. There's other points in the show where characters are shown transforming in real time. Just watch for it.
If they were ever attacked while transforming, I'm sure it'd be easy to react and dodge.. or in Usagi's case, fall on her ass while crying.

If you want a real mystery, here's one for you. If Galaxia knew that only Planet Guardians had Star Seeds, and was able to locate Mamoru's so quickly, plus is able to kick THAT MUCH ASS... Why was she even bothering to send out her obviously incompetent Sailor Senshi out after random targets.

OK, Nehelenia. She wants the golden crystal, and she wants to preserve her beauty. More than that, she want's revenge on the White Moon Princess for sealing her away. She gained her power by succuming to the power of the Dead Moon Circus. However, when we see the show, she is queen of the Dead Moon Circus. How the hell did she get to be queen of an obviously greater force that made her so corrupt? It seems to me that Zirconia should have been controlling HER instead of the other way around.
And then of course, there's the matter that in Stars, she says she created all of the monsters in the Dead Moon by eating the dreams of her servants. If that's what the Dead Moon Circus is, how could it have existed beforehand to tempt her to evil. Seems to me that the Dead Moon is more or less just a creation of hers, and maybe she's really just demented.
Whatever the case may be, there's a lovely plot hole there.
