I've never messed with Blu-ray discs or looked much at the specs, so I'm completely in the dark about how they go about dealing with resolutions and ratios - I'd assume that the specs would err on the side of 1280x720 and 1920x1080 only (with possible exception for anamorphism if that even applies), and any 4:3 content would require pillarboxing.

Alright, well that was the question, how exactly can you do it WITHOUT screwing it up. And the answer is found out. There is no way to make pure 720p footage from a standard dvd source. If you want to make an amv at 720p, you will have to either:
1) upscale the dvd to 720p and deal with the crappy quality...
2) purchase the hd-dvd or bluray of the source and use that...
