my AMVs and i want to enter to this contest ^^u

I want to know if you can provide me this information;
I visited the web that Jenny-sempai provide us and i have a question!

2) The AMVs information.... i mean...what kind of information??
it´s the first contest that i want to participate, so, i don´t know
much about this....

the one listed right down that point:
a. The name of your video(s)
b. The name of the series from which you are drawing your visual material (what anime you used in simple words lol)
c. The name of the song you are using, as well as the original artist
