Daa! Daa! Daa! episode 2. Although Kanata is supposedly brushing his teeth, he's more like walking around holding a cup and with a toothbrush in his mouth.

Clannad the movie, there's a scene where both Okazaki and Furukawa are brushing their teeths together.

True Tears, there's a scene in one of the episodes where Shinichiro goes and brushes his teeth, but mistakes the tooth paste for something else and ends up brushing his teeth with something that wasn't tooth paste. That leads into a scene where Hiromi watches him and laughs, then takes a picture.

Tenchi Universe episode 26, Tenchi is brushing his teeth before his dad calls to him and if I can recall correctly, Spike was brushing his teeth in an episode of Cowboy Bebop but I dunno which episode.
