Ok, so i have GTO in dvd form, right?
So i'm ripping them to my hard drive to use for an AMV, and for some bizarre reason, all of the volumes work except volume four. it just absolutely refuses to be read by my DVD drive;
a Toshiba SD-R2512.
The dvd in question works perfectly fine on my ps2 and dvd player, but won't read in my laptop's dvd drive. wtf?

go to www.cdfreaks.com and check for the latest firmware/drivers for your drive.

Very good call there bum, you might also want to try The firmware page. But I've got a feeling it's more or less the same site, but a different layout, oh well, give it a go.

I have a dvd like that. Try ISObuster:
http://www.smart-projects.net/isobuster/
Usually it will read the dvd after a few tries, and once it's 'open,' the dvd will register in other programs long enough to rip it.
