Read through this page VERY CAREFULLY and follow all of the instructions.
If you've alread gone through this process and uploaded your video, maybe you need to go to the Confirm Upload page. The link to this should be on the Left side of your Members Main Page.

There should be a line near the bottom that says "Choose a video to upload" and then a dropdown menu that lists the videos you have in your profile (and it looks like you only have 1).
With that selected in the dropdown menu, click the button below it that says "request upload permission"
If it's not listed there, your problem could be that the Anime source you have listed "DBZ 07" is not listed as an official/confirmed anime. You probably meant to list DragonBall Z: Movie 7. Had you selected this instead of just entering in "DBZ 07" then it probably would be working fine.

I read through this and when I went to the page all i got was:
Request Video Upload Permission
All of your videos are already locally available. Either you have already uploaded all the videos that qualify for uploading or you have not entered any videos on your profile.
Go to your member's main page
click on "Enter a new video" and fill out all of the information that applies.
After that, click on "Uplaod video".
Follow the instructions and it should work fine.
When using a FTP program make sure that your firewall is low enough to allow file transfering.
