Coincidentally enough, I watched Tomoe in the Moonlight just yesterday.

Awesome video, and it gives me a few ideas.

Okie day...I have NO IDEA about the first question...and all I know about the section question is, that I have heard all piano instrumental songs...like I used an all piano version of Numb (IT'S SUPER PURDY!!!) in my AMV "Welcome Home~Numb" Dat's all I know...well, I know one more thing...the only reason I am reply to this topic is because of the fact that's it's 2:30am where I am and I have too many fudge brownies to go to sleep *twitch* *twitch*

ErMaC's Extraordinary World isn't a MAD, but it uses a good number of manga scans.

Piano music AMVs? Oh, yes, they're out there.

Pen-pen's advice holds true: overediting can kill a nice piano piece. Not every note has to be synched to something in the video, though you should take heed of the more prominent sounds like glissandos and such. Listen for moods and themes, and go from there.
