"(For groups, add additional "Signature" lines as necessary.)"

I saw two signature lines. I signed the first, because the text implied that the second was for a second person, and that additional lines would simply be needed to added if more than 2 people were to sign.

And it's very unprofessional to be mocking people for simple mistakes in a public forum.

Remember, people blamed the Florida absentee ballot when people used it improperly, not the people who filled it out. We could do likewise.
"If someone feels the need to 'express' himself or herself with a huge graphical 'singature' that has nothing to do with anything, that person should reevaluate his or her reasons for needing said form of expression, possibly with the help of a licensed mental health practitioner."

Is that what most people forgot? Oh well then maybe I didn't screw up. I read the instructions and they are slightly different (the clauses that is).

Vlad, I think the old age is messing with your vision. That or being Canadian :-P

You know Vlad, I could scan the second page and post the picture of it for everyone if you want.
I'm here to help ^_^

Don't feel too bad, there was someone who sent in a blank form.... the entire thing was just empty of any information at all.
