Well, literally it is when a train crashes and a giant jumbled mess is made.

When refered to in a thread, it means it has gone bad and off topic.

Other terms in Amiercan slang probably not covered in your dictionary either that describe a jumbled mess like "trainwreck" are clusterfuck and snafu.

Snafu is actually a military abbreviation for "Situation Normal All Fucked Up".

Clusterfuck is generally used when there is a huge group of rowdy people being loud, noisy, and disorganized.

In other words, the All About Me forum.

An alternate version of clusterfuck is fustercluck. Means the same thing, but the f and the c are reversed for personal preference, usually to avoid saying the word fuck, either because you don't want to or because the situation may be inappropriate (although everyone knows what you really mean anyway, so what the point is, I couldn't tell you).
