Naruto is a pretty weird anachronistic show.
It's got all sorts of stuff just sitting on street corners that shouldn't exist, in a period cartoon.
I'm pretty sure I've missed plenty of stuff, but here are some of my observations.
As a rule of sorts; if you don't know the episode number, try to mention when it takes place to that others can find the things you mentioned.

Internal combustion engines. (why does everyone walk everywhere if they have engines, implication being; cars?)
Early in the series when they are going to gaurd the bridge builder, they are on a boat with an engine. He doesn't use it until he has dropped them off, but it's there all the same.
Radio Comm gear. (why communicate through scrolls and couriers if they have radios?)
The first Ninja mission, and I think the only time it's seen, the crew all has radio comm gear while Kakashi gives directives to the target.
Phone lines, telephone poles, but no actual phones. (see above)
Seen throughout the show.
Medical epuipment (electronic)
During the Chunin exam, at some point we see a hospital room, with the typical heart monitor beeps coming out of it.

This is something I have also thought about. If you read the original pilot manga, the creator does indeed include cars and guns. If I had to guess why he left these out of the series, I would think it was to try to keep the mystic-ness of the ninja world in there while also having a tech presence.
Leaving out guns would make the need for fighting more important. If they had guns they would would just shoot each other instead of fighting. Cars would kill the need to walk which is a way to stay fit. Plus it adds to the time needed to get from place to place I suppose.
Of course this is all speculation, but in the end I like it the way it is.

I think the manga notes at some point that the world of Naruto is not far off of modern technology in terms of what's there, but that actual use of technology is limited. There's no real explanation given.

It reminds me of Shaman King--the Native Americans tend to have lots of "genuine traditional hand-crafted" items (a la the Oracle Pagers, which are very obviously electronic).
*shrug* My speculation is that--well, the entire world is not comprised of Ninjas. The Ninja countries only exist on one continent in societies nearly completely separate from the rest of the world. They're kind of like magic users in Harry Potter, really. They live in the same physical world, but have completely different systems of government and completely different ways of doing things, but the wizards are still affected by the muggle world (well, more so in the movies--Lupin has a record player, for example, and most of the characters wear normal clothes, not wizarding robes like Dumbledore).
Think of the ninjas in Naruto like that. As the outside world changes technology-wise, so the ninja world changes--it introduces stuff like cars and electricity and photography. The ninjas need some things and can do better on their own over others. They need electricity, but they don't need cars (except for the inevitable few who have no sort of talent whatsoever). They pick what they need from the real world and leave the rest alone.
