She died, but as I stated in another thread, she is also within the LCL goo. If you've seen the TV ending with Episode 25 it is made apparent that she is also going through instrumentality (all that self-searching mumbo jumbo). Not exactly. The reason Rei fell apart is because Shinji chose to reject instrumentatlity (for all of mankind). I know that each of us has our own choice to come back, but Shinji was still the one in charge (as stated by Fuyutsuki). He rejected an ABSOLUTE instrumentality (where we would have no choice) and instead we leaked out of her neck and back into the ocean. In these circumstances, we could choose to come back in physical form (like Shinji did) or stay within the LCL.
