First off... I am almost done with the admin interface... i will begin testing files when that's done... I made the user side first (your side) since it was more important
the admin side is more complicated than i thought...
second... make sure that when you fill out your personal information... you fill it all out... alot of people are not inputting thier phone numbers, nor thier mailing address...

for fanime... whats on the website is ok.... no need for a seperate form...
and... i just updated the website...
man my skills are pathetic... but... i just added a nice feature at the top which breaks down the steps needed to submit a video... and hopefully cleaned up the interface a bit... and add actuall explinatiosn of what everythign means....
