So for fields that can't be filled in via Acrobat (the YES and NO fields for age verification) and the signature, those are ok to leave blank?

I opened the PDF in Photoshop and filled the fields with the text tool, and scanned a copy of my signature.

I printed the whole thing once all fields were filled - made a mark on yes and signed it and then scaned it as a pdf and send by email.

I would recommend that someone from AWA make the original PDF editable. Or perhaps they want to force a hand written signature and that's why they didn't make it save-able from Reader.
For those who don't know when you have text field/form data in the PDF by default most apps that generate the PDF for you don't have it setup so Reader can save the file after you enter stuff. Basically so Reader can save it you have to open the PDF in Acrobat Pro and enable usage rights.

So yeah, I'm a wuss and decided I'd send my entry in to the expo instead. I'm just not sure I'm up for competing at a pro level.
Even though every AMV I've entered anywhere has been a finalist and/or a winner. And I did win at Otakon last year. Meh, now I'm kind of feeling bad about entering expo now.

Ignore the appellation "Professional". We're all amateurs here anyway.
The real difference between the Expo and Pro contests is not the level of quality of the competition, but how they're judged.
Expo's always been kind of a showcase of everything they can get their hands on from the past year, with the awards being decided by the VAT staff.
Pro on the other hand is judged by the contestants themselves, for an AMV contest experience you won't get anywhere else.
If you've seen the Expo winners before, you know how good the videos that get sent to that contest every year are. Remember, "Shameless Rock Video" (2002) and "Euphoria" (2003) were both entered into the Expo contest, and who knows how many others of similar stature.
I hope that made sense.

Pretty good wrap up by Scintilla. The only bit I'd elaborate on is that it's not just VAT staff but anyone else we can grab. It makes for a fairly diverse and exotic cross-section for our judging panel. But the Pro contest allows you the rare opportunity to be judged exclusively by your peers.
It's better that you're having second doubts now rather than after the deadline. If you want to switch your entry between contests, just say so and fill out a copy of the Pro form.

Hey guys, is it allright if I upload sunday? I got the video lsited on the vat page but the FTP timed out and I'll be at comic-con till then and am going to have to cut it close. I'll try to get hotel internet but the bandwith and ability are questionable at best.

I don't understand how AMV creators are able to submit new videos this year. There was the incredible July steam sale which dep-sixed Pro and Expo is supposed to be totally screwed because of Starcraft 2 next week. All this talk of new entries makes me think this AWA won't be a "best of" AMV entries from 2010 Spring and Summer conventions.
