Is it opening Outlook when you click the button? You don't have to use outlook, just send the FDF file as an attachment via Hotmail/Gmail.

Edit: I'm in contact with the creator of the form about the problems. I'll update with fixes. I'm very sorry about the trouble.

Sok, let me know when it's fixed... maybe it's a little bit of my incompetence too. XD (...I have no idea what an FDF file is... O_o;; ) And yeah, it kept opening Outlook. And my only email is AOL...

I sent it to whatever the PDF "Email" button posted. And I checked if it matched the real email and it was the same.

I would be sending the PDF as an attachment through Yahoo, but the PDF won't let me save it once I open it and fill out the information (only send it through that email button). =/

Ok, for those who are having problems with the entry form here are some options.

HTML Version of the Entry Form

Just fill it out and Save As PDF or edit in in a word processor and e-mail the file.

All I need is the FDF file the form generates. I'm sorry about all this trouble, but you may need to send it again as I have not received anything. Send it to banzaiamv@gmail.com as well.

How do I save it as PDF? Everytime I say Save Page As, all it gives me is is Web Page, Complete, Web Page, HTML and Text Files...and I've tried copying and pasting it to microsoft word and the thing keeps freezing.

Yeah, that's what it saved the attachment as.

Had to go through a roundabout way, saving the attachment it was trying to send since it wouldn't let me save an FDF file in the first place, bleh.

K, sent it to both emails just in case.
