Problem with update video anime form for VCAs?

Is there supposed to be such a huge list of pending requests, sometimes with 6 or 7 of the exact same request, which I KNOW I did not make, showing up when I try to add two series to one of my videos using the VCA 2004 form? I submitted my request a few days ago, then re-visited the form yesterday and saw that neither of the series I'm trying to add was on the huge "pending" list. Did something go wrong and prevent my request from being processed?

And is anyone else having this problem?
