the hostname is kero.animemusicvideos.org
to get the username/password you need to click "upload video" in the editors menu (or click here).
You need to have a confirmed anime in your video information to be able to upload it. MM! isn't confirmed, so you won't be able to upload anything for that video in your profile until it does get confirmed. Mirkosp and Vlad usually take care of that
