No Radio Button on "Confirm Upload" page

Hello,

I'm trying to upload one of my videos locally. I went through the usual steps of selecting the video to upload, and then ftping. When I go to the confirm upload page, it has the title of video, but where I should be able to click it to test download it, it's not a link. And there is no radio button to select to choose "confirm upload", although there is a confirm upload and a cancel button. I have tried 2 different browsers with the same results, and I tried cancelling the upload and doing it all over again, but I still keep getting stuck at the confirm upload page. I have never had this problem before.
