Premiere definitely doesn't use directshow. Maybe for the previewing, but I'm not even sure of that. I don't know enough about how it handles interframe codecs to tell you what makes it frame inaccurate, I know it shouldn't be at least with I and P VOPs. B VOPs do different strange things with vfw depending on how they're stored, usually it's a lag frame or two.
