problem during crossfade

I use Adobe Premiere to edit with, and whenever I use a crossfade, some weird horizontal line appears in the middle during the fade. its always there during every fade, in every video I work on. was hoping maybe someone might be able to tell me what I'm doing wrong.

How is your source encoded?
