Export lossless from your editing software then use AMVtool to convert it, the default settings should probably get you the size you need, if not try changing CFR from 18 to 20.

There should be a warning that the AVIsynth Premiere plugin only works on CS 5.5 and older. Newer versions of Premiere require some kind of file system trick that I haven't been brave enough to experiment with.
