Why is your video 1920x1200????

Let's try to work all the way through this.
Your source footage that you are putting into premiere is 848x480??

When you export from premiere, it should be the same. You should be exporting at 848x480, 23.976 fps, Pixel aspect ratio 1. No black bars or anything should be added to your footage. If anything is changing between your source and your exported footage, the problem lies here.

Assuming the size and everything is correct at this point, you should be able to load it directly into tmpgenc and encode from there. (you aren't doing any complex stuff in the avisynth script, so lets just skip that to simplify things)
In tmpgenc, under the video tab, settings should be as follows:
stream type: mpeg-2
size: 720x480
aspect ratio: 16:9
framerate: 23.976
...
Encode mode: 3:2 pulldown when playback
Motion Search precision: highest quality

Under the Advanced tab, settings should be as follows
Video source type: progressive
field order: bottom field first
source aspect ratio: 1:1
positioning method: full screen


Now, if something still screws up, you need to let me know at exactly what point its getting screwed up.

Alright, I figured out one thing: before I was exporting from Premiere in 720x480, and now I got it to export in 848x480

However, as you can see there is still some small issues

The reason those images were 1920x1200 is because I took those pictures in full-screen on my 24" monitor running in 1920x1200 resolution

Here is the export from Premiere in full-screen (you can't see it here, but on the top and bottom there's big black bars like if you're watching a wide-screen movie on a 4:3 television, and then very small black bars in the actual video):

And here is the export from Premiere not in full-screen (I tried to get it at 848x480, but the printscreen wouldn't get the whole picture, so I went with 1280x720):

Ok, then from the looks of things, we now have the output from tmpg looking exactly like the output from premiere, correct?
Its just that premiere is still screwing something up?

For the most part, it looks like the mpeg2 file is mostly correct, it just has some fairly small bars on the sides. I would say that it should be acceptable at least, if you can't work out whats causing it to mess up in premiere. Do the bars around the sides happen to exist in the source footage?
Go back through premiere and check all your export settings. Or in fact, your project settings may be weird as well. I'm not really sure if the project settings have any affect on the export or not.

Well I'm not sure if its because of the initial 848x480 setting or what (although premiere seems to think the source is 720x480?)

From what I could tell the Project Settings have no bearing on what gets output

Ah yes, it would seem you messed it up in the beginning.
What you can try doing is create a new sequence with the correct settings, and then drag and drop the old one into it. That might help. In any case, I wouldn't worry too much about the small about of black borders that are left. It really looks like no big deal.

Well I tried to create a new project and edit the resolution, but all I could find were "presets" and there's no way to pick out a specific resolution as you can see here:

On that tab to the left there's just a bunch of preset choices, so there's nothing I can do?

Yes, those settings can be modified right there.
First you change the editing mode to desktop
timebase to 23.976
frame size to 848x480
pixel aspect ratio to 1.0
fields to progressive

HOWEVER, changing the timebase may effect some of the cuts you have made in the video, so you should go back through and carefully check everything after making changes.
