Greatest could mean anything to you: most dramatic, least expected, coolest, most painful, most emotional, most satisfying, etc.
Needless to say, this thread would be full of spoilers, so what I suggest would be to put the name of the anime right above the spoiler so as to warn people whether they want to see or not:

[spoiler]I thought both L's death and Light's death were beautifully depicted. L looking up at Light's grin and slowly closing his eyes. The sunlight reflecting off Light's body on the stairwell as well along with the peaceful expression on his face as he slowly closes his eyes just as L had. It calms my soul.[/spoiler]
Fullmetal Alchemist:
[spoiler]Ed dies twice in one episode. Sucks to be him.[/spoiler]
Rurouni Kenshin: Reflection OVA
[spoiler]Kenshin died of AIDS. LOL
But seriously... it was a very touchy-feely moment. Kenshin limps toward home, weak from the disease. Kaoru senses him and meets him halfway. Kenshin died in Kaoru's arms... and THE SCAR FINALLY HEALED!
Code Geass:
[spoiler]Lelouch shoots Euphemia without hesitation. Suzaku pays witness and rushes to save her. He carries her bloody body to command. Euphemia is pale; there is beautiful music playing in the background. Suzaku tells Euphemia that the Japan Project was a success. Euphemia dies happily while the people she was trying to help curse her existence. Spectacular.[/spoiler]
GANTZ
[spoiler]This just sickened me. The group walks into the room and finds their comrades' torsos embracing eachother, with the other halves on the other side of the room. Oh my God! What happened to them? Then we find out. Kishimoto jumps in front of an acid attack that is meant for Satou. The acid lands on her back and slowly rips through her suit, spine and flesh, all the way through the front of her body. Mind you, this is one of the three main characters: people who's story has been built up and had character development. These people have things to live for. I almost threw up when she died. Not to mention that Satou, the person she died protecting and another main character at that, died at the end of the chapter, too. In fact, everyone died. There is only one person left at the end of the third chapter. What the hell? Still it deserves "Most Gruesome Death" in my book.[/spoiler]
