Just for testing, create a new project and begin by importing this clip just to check that your avi file is dead.
To restore your project, you really do not have a lot of choice from what I can see.
- You need to be skilled in XML or html... Open your project file in a text editor such as Notepad and rewrite the corrupted part by analysing the code and comparing it to another project. (yeah I know, It is going to be hard...)
- If losing some info from the last time you saved do not bother you, you have a really easy solution. In your project directory, go in: "Adobe Premiere Pro Auto-Save" and open the last modified project. After that, do not forget to do a Save as... to save your own file.

As for the clip, have you tried encoding it to another video format and seeing if that works. Not a Mac user here so I don't know what formats are available. I'm sure Uncompressed should.
And if there really is an error in the code then an complete re-install should re-work the code and possibly alleviate the problem without harming your project. I've done it a few times myself.
I've also heard of people who had problems and then one day it's gone. I think Adobe programs have minds of their own.
