It's a virtual reality game, and Tsukasa fell into coma while playing it. When that happened, Tsukasa's mind was trapped there.

There are instances where other characters begin to experience the same things. Mimiru, for example, notes that Bear's hand feels sweaty as he pulls her up when they explore the Upside-Down world. The storyline of the Video Games have characters that fell into comas still playing the game in the system (Kite sees Sieg around, even though his player is in a comatose state in LIMINALITY).
The reasons behind the comatose states aren't revealed until Legend of the Twilight.

The "old man" (aka Harald Howerick, the creator of the game) was trying to tell the truth behind Aura and Morganna Mode Gone, but the data it used was very fragmented and could only be understood by those who had seen Aura (like Tsukasa).
Sora's fate is even more dire. In the last episode, he fails to escape Morganna Mode Gone and his character data is used to create Skeith, while his player (a 10-year old kid) goes into a coma. Skeith returns in .hack//INFECTION as the first boss you fight against. Once defeated, Sora is freed from him and, in .hack//OUTBREAK, you will receive Sora's Blades as a reward (the best blades, IMHO - they can cause Instant Death).

The most interesting part of The World game is that it copies the personality data of everybody. This is because Harald Howerick wanted to give Aura a broad range of human experiences. However, that data gave awarness to Aura's "nanny", Morganna (aka The World's Operating System), who rebelled against Hoerwick's programming and kept all of Aura's power for herself. Therefore, the "memories" that Tsukasa has are just "personality data" collected by the system and under the manipulation of Morganna Mode Gone.

Semi-spoiler reply wrote:I thought that while the core cause was the same (as in an evil semi-omnipresent being in the World), the reason for doing so was different between Legend of the Twilight and SIGN (I haven't played the games) In LotT, it is so said being can bring True death to the world and discover it's secrets, but it's different in SIGN.
