Help confirming

I uploaded my AMV to the server. I go to confirm video, and the file name is correct, but the video name above it says the name of my other video, and it even links me to it. If I try to upload this new video I have bad feeling it's going to replace my old video. How do I add a NEW video without replacing old ones..? I'm stuck.
