Congratulations, you've just discovered the wonderful evil that is interlacing.
If your source footage looks like that, then you need to IVTC or deinterlace (IVTCis vastly preferable). Otherwise, you need to make sure that you've selected "no fields". If your source is clean and Premiere is interlacing your video when you tell it not to, then your computer has become posessed by Satan and must be melted down.
Also, if you're exporting in HuffYUV or with no compression at all, then that explains the choppiness... most computers can't play back huffies or uncompressed video without chopping a little (and you shouldn't be watching those files anyway, since they exist solely to be put into VirtualDub, converted into something smaller, and deleted)
As for the macroblocking, well, that just plain shouldn't happen with huffies or uncompressed video. Again, posessed by Satan, melt down, etc.
